SAMPLE CANCELLATION FORM

Under the new law, consumers must be informed about the model withdrawal form before placing an order. This form must not be confused with the cancellation policy and must always be used, regardless of the information provided. The withdrawal form must be provided to the consumer on a durable medium. It is recommended that it be placed below the cancellation policy so that the consumer receives both together in one document.
